Excellent article on writing a performance review. I recommend that you create a Documentation File when you start a new job or at the beginning of a new review period. In this file, record all achievements, future goals, new skills that you have learned, and any other communications that compliment your performance. When the performance review takes place, you will be able to provide documented evidence that you are meeting or exceeding expectations. Sometimes, performance reviews are tied to a salary review so you want to make sure you are well prepared to discuss past performance and future goals.
